Dr. Ronald Goldenberg is a consultant endocrinologist affiliated with North York General Hospital in Toronto, Ontario and LMC Diabetes & Endocrinology in Thornhill. His major areas of interest include clinical care of diabetes, obesity, dyslipidemia and thyroid disorders. Dr. Goldenberg is a past Medical Director of the Diabetes Education Centre at NYGH and North York Branson Hospital. He is a past chair of the Ontario Medical Association Section on Endocrinology & Metabolism and a previous President of the Toronto Diabetes Association. Dr. Goldenberg is a councillor on the 2010-2014 Canadian Diabetes Association Clinical & Scientific Section Executive Committee and a member of the Steering Committee for the 2013 CDA Clinical Practice Guidelines, the Executive Board of the Canadian Society for the Study of the Aging Male, Canadian Diabetes Association, American Diabetes Association, Endocrine Society, American Association of Endocrinologists, European Association for the Study of Diabetes, Canadian Society of Endocrinology & Metabolism and a Fellow of The American College of Endocrinology. Dr. Goldenberg has been an investigator in a wide array of clinical trials in the areas of diabetes, hypertension, obesity, and dyslipidemia. He has been actively involved in Continuing Medical Education for the last two decades and is a key opinion leader amongst community-based endocrinologists in the Toronto area. He has been awarded an Honorable Citation by the Medical Staff Association of North York General Hospital for excellence in the practice of endocrinology and dedication to patient care in the community. Dr. Goldenberg completed his residency in Internal Medicine in 1987 at the University of Toronto, and his fellowship in Endocrinology & Metabolism in 1989 at the University of Toronto.

  • Why Am I So Tired? : Hypothyroidism

    Sarah is a 32 year-old software engineer who presents with a one year history of fatigue, difficulty losing weight, dry skin, cold intolerance and difficulty concentrating. She has been busy working long hours and initially attributed her symptoms to long work hours and stress. Her mother has a history of hypothyroidism.

    At her routine follow-up with her family physician, Sarah mentioned her symptoms, and her physician noted slight enlargement of her thyroid gland. The results of her blood tests were as follows:

    Thyroid stimulating hormone (TSH): 15.6 mIU/L (N 0.32-400 mIU/L) Free thyroxine (Free T4): 7 pmol/L (N 9-19 pmol/L)

    The combination of signs and symptoms along with bloodwork showing an elevated TSH and low Free T4 confirmed the diagnosis of hypothyroidism, almost certainly due to Hashimoto’s thyroiditis, the most common cause of hypothyroidism and typically more common in women than men and often associated with a family history of thyroid disease.

    Sarah is started on thyroid hormone replacement therapy with a high quality brand preparation of levothyroxine (Synthroid®). Her pharmacist instructs her to take the medication on an empty stomach every morning. Within about 6 weeks, all of Sarah’s symptoms have resolved and she has returned to feeling perfectly normal. Repeat bloodwork done 8 weeks after initiating treatment revealed a normal TSH.

    Sarah will benefit from regular follow-up with her family physician, with TSH testing at least once yearly or if symptoms return. Her pharmacist can help with ongoing reminders about how to properly take her medication.

  • What is Adrenal Insufficiency? " Mary is a 44-year-old who presents to her physician with symptoms of poor appetite "

    Mary is a 44-year-old journalist who presents to her physician with symptoms of poor appetite, nausea, lightheadedness and abdominal pains. These symptoms have been going on for the past year, and she has also noticed a 20-pound weight loss during this time period. Mary does not spend much time in the sun, however she has noticed some changes in the pigmentation of her skin.

    After review with her family physician, routine blood work shows that Mary has low sodium and high potassium levels and she is referred to an endocrinologist. Further investigations lead to a diagnosis of Addison's Disease (primary adrenal insufficiency), and Mary is treated with glucocorticoid and mineralocorticoid hormonal therapy which completely resolve her symptoms. She is also given instructions on how to adjust her medication when she is acutely ill and how to maintain proper salt and fluid intake to avoid dehydration.

    Mary would benefit from continuing to work with her family physician and endocrinologist to manage her Addison's Disease. She may wish to work with a dietitian to understand the impact of her nutritional intake on her hydration levels as well as her vitamin and mineral intake. A pharmacist could help her manage her medications and supplements and avoid any adverse interactions with her prescribed medications. Finally, keeping stress levels down is important for patients with adrenal insufficiency. Taking yoga or getting a massage may help Mary in her day-to-day life.

  • Pituitary Gland Tumour - Effects on Health " Arlene is a 28-year-old graphic artist  "

    Arlene is a 28-year-old graphic artist who has been noticing changes in her menstrual cycle recently. She visits her family physician to share her symptoms which include the absence of her menstrual period for the past 6 months, as well as occasional discharge of milky fluid from her breasts. She is concerned by these symptoms, and also wonders if this is affecting her fertility as she hopes to conceive in the near future.

    Her family physician orders some blood tests and discovers that Arlene's prolactin level (the hormone that produces milk) is 10 times the normal level. She is referred to a local endocrinologist, and a subsequent MRI scan of her pituitary gland shows a lesion which is 1.3 cm in size. Arlene is diagnosed with a benign (non-cancerous) prolactin-secreting tumour of the pituitary gland, and she is started on oral medication to treat this. Shortly after starting treatment, she experiences a decrease in the breast discharge and her menstrual period resumes.

    Along with routine follow-up and monitoring from her endocrinologist and family physician, Arlene would benefit from having a detailed vision exam, as pituitary gland tumours can press on the optic nerve and create visual symptoms. She should also consider lifestyle changes to help with her ongoing treatment and to prepare her body for pregnancy, such as quitting smoking, limiting alcohol intake, and prenatal yoga. She may wish to consult with a naturopathic doctor or a registered dietitian to learn about vitamins and supplements that could support her health goals, and a pharmacist to understand any possible interactions between vitamins and medications that she is taking.
