Dr. Elliott is a well-recognized Canadian expert in the field of male sexual dysfunction.Dr. Stacy Elliott , Psychiatrist, Vancouver is a Clinical Professor in the Department of Psychiatry and an Associate member of the Department of Urologic Sciences at the University of British Columbia. Dr. Elliot is the Co-Director for the Vancouver Sperm Retrieval Clinic (VSRC), the Sexual Medicine physician consultant to the Sexual Health Rehabilitation Program at GF strong Rehabilitation Centre. Bipolar disorders are a type of mood disorder. There are several subtypes of bipolar disorders. The most common ones are Bipolar 1 and 2 disorders. These affect approximately 5 percent of the population, which means that we have about 1.5 million people with this condition in Canada. Bipolar disorder is a serious psychiatric condition.

We all forget things as we age, but that doesn’t mean we have dementia. Dementia isn’t just one condition - it’s a term that refers to symptoms that cause a decline in mental ability that interferes with daily life.   • Erectile Dysfunction Following a Vasectomy " Jason a 42-year-old executive with three children"

    Jason is a 42-year-old executive with three children. Although both busy with their careers, Jason and his wife had a good sexual life, and he had no erectile or ejaculatory difficulties. He recently underwent an uncomplicated vasectomy, and when he resumed sexual activity, he was disappointed that his erection was not as firm and that the orgasm he experienced was not as strong. He blamed it on a diminished penile sensation since the vasectomy, and dismissed it as temporary.

    Unfortunately, this issue continued and several months later, Jason visits his family doctor. His physician reassures him it is not the vasectomy per se, and tells Jason this should resolve itself as his psychological outlook improves. However, Jason requests a referral to a urologist for a vasectomy reversal.

    The urologist repeats what the family doctor had said, and feels that Jason should see a sexual medicine specialist or a psychologist as there is no organic or medical cause for his sexual changes and does not want to do a reversal without further assessment.

    Jason sees the sexual medicine physician who re-explains anatomy and introduces the idea of how the mind-body connection can affect sexual function and sexual signals to the pelvis, and what the vasectomy meant to him in terms of manhood and stages of life. A 3-month trial of daily PDE5i is introduced to improve nocturnal, spontaneous and sexual erectile function and improve pelvic vasocongestion. Jason also sees a sexual therapist for three visits. Over time his erectile function and orgasmic quality improves substantially, although not to what he feels are pre-vasectomy levels.

    Jason could benefit from continuing discussions with his family doctor and urologist to monitor his physiological symptoms. He could also continue seeing the sexual medicine specialist and perhaps the sexual therapist to work on any psychological component to his condition.

  • Testosterone Levels and Erectile Dysfunction " Harry is a 62-year-old man who is 30 pounds overweight "

    Harry is a 62-year-old man, who is 30 pounds overweight and has Type II diabetes currently managed by Metformin. He has noticed diminishing erectile function over the last 3 years but now he has generalized dysfunction, unable to get an adequate erection with his wife or with self-stimulation. He has also lost morning erections and has difficulty ejaculating. His wife complains of his irritability and diminished sexual drive, and is becoming sexually disinterested herself.

    Harry's family doctor gives him a phosphodiesterase V inhibitor (PDE5i), which does not provide an adequate erection for penetration despite higher doses and several tries. Recognizing his symptoms of low testosterone and the failure of the PDE5i, he is found to be hypogonadal, and begins testosterone replacement therapy (TRT). While this improves his libido, fatigue, mood and improves his ejaculatory capacity, it does not improve the erectile dysfunction (ED). However, with eugonadal serum testosterone levels, the PDE5i starts to work and Harry is able to resume sexual activity without proceeding to the vacuum device or intracavernosal injection for erectile dysfunction. The improved energy allows him to become motivated to exercise and lose weight, allowing his TRT to be discontinued. However, he still requires the use of the PDE5i.

    To assist Harry with continued success dealing with his testosterone levels erectile function, he would benefit from working with his family doctor, an endocrinologist, and perhaps a urologist. He could also consider consulting a sexual medicine specialist. Harry could also work with a registered dietitian and an exercise physiologist to help him with his weight loss efforts.
